I agree in an emergency, if I cannot be contacted, I am willing for my child (named as above) to receive any hospital treatment, including anaesthetic if necessary. I understand that every effort will be made to contact me as soon as possible. I understand that CCYFC and its volunteers do not act ‘in loco parentis.’ If I choose to leave my child unattended I do so at my own risk. Parents of players in Nippers and Under 6 squads must stay with their children whilst they are at training. I understand that CCYFC and its volunteers limit their liability (to the fullest extent permitted by law) in respect of any personal injury, loss or damage to players and/or their property while attending or travelling to or from training or matches. I have read and accept CCYFC’s Club Handbook and Associated Policies I agree to CCYFC retaining personal data for the purpose of membership of CCYFC and league registration. The Club holds the data on a secure database, and it is the responsibility of members and or parents to keep the data up to date.
